What is My UNC Chart?

UNC Health Care offers patients personalized online access to portions of their medical record. My UNC Chart enables you to securely use the internet to help manage and receive information about your health. My UNC Chart may be used to:

  • Request medical appointments
  • View your health summary*
  • View test results*
  • Make payments for services*
  • Request prescription renewals*
  • Access trusted health information resources
  • Connect with your providers to ask a question about a non-urgent need*
  • Complete your medical history and questionnaires prior to your appointment

*Also accessible on the MyChart app available for iOS and Android.

Is there a fee to use My UNC Chart?

How do I sign up?

1. Visit myuncchart.org.
2. Locate the "New User?" section of the website.
3. If you don't have an activation code, click on "Get Activation Code." If you have an activation code, click on "Use Activation Code."
4. Once you follow the steps in either the Sign Up or Activate Your Account process, you will be able to create a username and password to sign into your account.

When can I see my test results in My UNC Chart?

Releasable test results are made available in your My UNC Chart account within 1-7 business days. Narratives are automatically released in 14 days.

Are all test results shared electronically by way of My UNC Chart?

We believe that all test results should be shared with our patients and many of these results can be viewed electronically in your MY UNC Chart account. Some tests are of a sensitive nature and require careful and thoughtful conversation between you and your provider. These types of tests results will be shared with you in person.

Not all test results can be seen in My UNC Chart. For those tests, My UNC Chart will only display the date completed with no additional details on the findings. This is so that you will be able to know that it was resulted.

If I find incorrect information in My UNC Chart, what should I do?

Your health information is reviewed and updated in your electronic medical record each visit. Ask your doctor to correct any inaccurate information at your next visit.

With My UNC Chart you can request the addition or removal of Current Health Issues, Medications, and Allergies.

1. Sign into My UNC Chart.
2. From the top menu select "My Medical Record" and then "Health Summary."
3. Click the section you wish to modify (Current Health Issues, Medications, Allergies).
4. To remove: Hover over the box and click on its “Remove” button. The item removed will appear at the bottom.
5. To add: “Add A Current Health Issue (Medication, Allergy)” button and type it in the box. Complete the information and click “Accept.” The item you wish to add will appear at the bottom..

Can I access another individual's health record in My UNC Chart?

Yes. Access to another person's health information in My UNC Chart is called "proxy" access. Proxy access allows an individual to view certain information of another UNC Health Care patient. To find out how to gain access to another person's account click here.

Can I ask questions regarding a family member from my account?

No. As written in the Terms and Conditions for My UNC Chart, your account must be used only to inquire about your health. If you have proxy access to another person's account, you may ask questions about that family member using your proxy access. To learn how to access another person's account click here.

Can my spouse and I share one My UNC Chart account?

No. Due to the sensitive nature of medical information each adult must establish his or her own My UNC Chart account. To learn how to access each others's account click here.

I forgot my password. What should I do?

You may click the "Forgot password" link on the sign-in page to reset your password online. You may also call UNC HealthLink at (888) 996-2767 for password assistance.

I've lost or misplaced my activation code. Can I get a new one?

Yes. Please follow these steps:

1. Visit myuncchart.org.
2. Locate the "New User" section of the website.
3. Click "Get Activation Code."
4. After completing the sign up form you will receive a new code at the email you provided.

Where can I update my personal information?

Sign into My UNC Chart and from the top menu select "Preferences." Under the header "My Account" and choose "Demographics" to update your personal information. Those changes are then saved to your electronic record.

What has UNC Health Care put in place to protect my data?

UNC Health Care is very conscious of protecting the security of all patient information. We have taken steps to ensure the confidentiality of your accounts. To ensure that communication between patients and My UNC Chart is secure there are a number of security features in place:

  • Robust technology is used to prevent unauthorized access to the system.
  • A security team constantly monitors the system for suspicious activity and automatic alerts are generated if any abnormal activities arise.
  • The latest data encryption technology is used to provide some of the best protection available to keep your password, account details and other sensitive information from being accessed by an unauthorized party.
  • A short time-out limit has been set. After this time, the system automatically signs you out and ends your session. Remember, if you are not using your computer for a period of time, please make sure you 'sign out' completely so that your information cannot be viewed by others.

What can I do to ensure the protection of my account details?

Abide by the following checks to ensure your account details remain secure:

  • Ensure that you regularly update the anti-virus software on every PC you use.
  • Make sure anti-virus software targets spyware as well as other viruses.
  • Ensure your browser connects at 128 bit encryption.
  • Never disclose your Access details to anyone. Do not write them down or store on your computer.
  • Select a password that is difficult to guess and change it regularly.
  • Never leave your computer unattended while signed in to My UNC Chart.
  • Always sign out when you have finished your My UNC Chart session.

What are Hoax Emails?

Hoax emails-'Phishing'-is a scam that uses fraudulent emails to entice people to surrender their sign in information. The emails are crafted to look like a legitimate email and often contain links to authentic looking, but fraudulent websites.

UNC Health Care will NEVER request account or password information from you via email. If you receive an email requesting this sort of information and it appears to be from UNC Health Care, delete it and report it by contacting us immediately at (888) 996-2767. Do not reply to the email.

I was signed out of My UNC Chart. What happened?

We aim to protect your privacy and the security of your information. While signed into My UNC Chart, if your keyboard remains idle for 20 minutes or more, you will automatically be signed out of My UNC Chart. We recommend that you sign out of your My UNC Chart session if you need to leave your computer for even a short period of time.

After you've created an account on the website, you can use a mobile device with an iOS or Android operating system, which both support the My UNC Chart mobile application.
